With more than 25 years of combined social care experience, we are here to guide, encourage, and empower individuals to get back their confidence and the skills they need to lead an independent and fulfilling.
Our work experience has afforded us the opportunities to develop therapeutic relationships with individuals, GP practices, community services, and inter-professional and inter-agency collaborations; these will facilitate the engagement and delivery of services.
We are also experienced and adept at liaising with external agencies, professionals, and internal and external stakeholders. We have extensive experience assessing needs, creating support and action plans, risk assessments, and monitoring and reviewing processes to support the recovery of individuals and support their aspirations.
